Anyone can make a referral to Hospice. If you think you or a friend, neighbor, or family member might benefit from our services, please call for confidential, no-obligation information. Admission of hospice-appropriate patients is possible within 24 hours of referral; usually the same day.

Hospice services are available 24 hours per day and 7 days per week. Patients/families in the Hospice program have round-the-clock access through an on-call number.

Regular office hours are Monday thru Friday, 8:00 a.m. to 4:30 p.m. The main and Pataskala phone number is answered by voice mail after hours and on weekends.

Location & Directions - Our main office is located at 2269 Cherry Valley Rd., Newark, Ohio 43055. Cherry Valley Road is on the west edge of Newark between West Main Street and Rt. 16. The public is welcome to visit the Hospice office and/or the Selma Markowitz Care Center. It is suggested that you phone ahead if you wish to arrange a personal tour.

Our Pataskala branch office is located at 621 West Broad Street, Suite 2A, Pataksala, Ohio 43062. It is on the west edge of Pataskala next to the 84 Lumber Company.
